The Elder Care Program is an interdisciplinary team (IDT) including a geriatrician, social worker, clinical pharmacy specialist (CPS), licensed practical nurse and a registered nurse. The IDT focuses on the care of the frail and elderly members, with primary focus on Special Needs Plan (SNP) and high risk Medicare members.
Essential Functions:
- Participate in the weekly IDT meeting; based on outcome and findings make telephonic follow-up calls to coordinate any immediate care needs.
- Based on KPGA regional Clinical Quality Strategy, outreaches to members in specific populations telephonically to perform health assessment, improve gap closure, quality of care and clinical outcomes. Outreach efforts support improvement in HEDIS and Medicare Stars measures.
Qualifications:
Basic Qualifications:
- Bachelor of Science, Nursing (BSN) or equivalent related work experience
- Licensure as a Registered Nurse in the State of Georgia
- Minimum of 2 years experience in ambulatory/ outpatient care, preferably in gerontology
Preferred Qualifications:
- Masters of Science in Nursing
- Minimum of 4 years experience in geriatric assessment
- Prior experience in self-management education programs
- Geriatric Nursing Certification
