Provide integrated and coordinated case management services for the high risk and high utilizer populations in both inpatient and outpatient psychiatry and addiction medicine settings. Responsible for managing all facets of care and utilization of high risk psychiatric and addiction medicine patients.

Essential Functions:
- Initial Intervention (Following referral from Inpatient and/or Outpatient Psychiatry Services).
- Engages with high risk, high utilizing patient populations.
- Acts as a central point of contact for referrals from multiple departments.
- Provides assessment and evaluation of clinical status, environmental resources, mental and intellectual capabilities and networking skills.
- Planning of Services is based on assessment of both short and long term needs
- Tracks referrals and outcomes for ongoing quality management.
- Operates as a consultant for other departments.
- Coordination of Care Facilitates collaboration between Primary Care, Behavioral Health Care Services and Health Care Providers.
- Provides referrals to other departments; facilitates this process and may provide follow-up services as appropriate.
- Consults with families and other care givers.
- Provides service information, educational materials and resource information.
- Provides brokerage and advocacy when necessary.
- Facilitates scheduling of multi-disciplinary assessment team and may participate in assessment.
- Linkage with External Resources Provides linkage of member to community services.
- Establishes linkages and maintains working relationships with outside resources.
- Direct Patient Intervention Provides individual and/or group therapy for chronic, high utilizing patient population.
- Provides individual supportive psychotherapy with client and supportive family counseling when necessary.
- Participates in Intensive Outpatient programs as a team member.
- Coordinates, provides and/or facilitates informational classes, education and/or training in areas such as independent living skills, psycho-education.
- Provides crisis assessment and intervention.
- Consistently supports compliance and the Principles of Responsibility (Kaiser Permanente's Code of Conduct) by maintaining the privacy and confidentiality of information, protecting the assets of the organization, acting with ethics and integrity, reporting non-compliance, and adhering to applicable federal, state and local laws and regulations, accreditation and licenser requirements (if applicable), and Kaiser Permanente's policies and procedures.
- In addition to defined technical requirements, accountable for consistently demonstrating service behaviors and principles defined by the Kaiser Permanente Service Quality Credo, the KP Mission as well as specific departmental/organizational initiatives.

Basic Qualifications:
- Relevant experience in the following area shown below:
- Eating Disorders: Minimum two (2) years recent experience in case management and working with community resources, specifically related to the assessment, diagnosis and treatment of a variety of Eating Disorders. At least two (2) years experience in a mental health setting and experience working in Inpatient and/or Outpatient treatment services.
- Master's degree in social work or related behavioral health field.
- Current and valid LCSWor MFTlicense issued by the State of California Board of Behavioral Sciences.
- Knowledge of Psychiatry and Addiction Medicine patient populations in inpatient and outpatient settings.
- Knowledge of the complex systems of psychiatric care and substance abuse treatment at the community and county levels to include: County Welfare Services, Community Mental Health agencies, residential care services, access to board and care services, Medi-Cal and Medicare regulations and Conservatorship laws as they apply to Service Area.
- Thorough knowledge of psychiatry and/or addiction medicine-assessment, diagnosis, treatment modalities, psychopharmacology, and institutional care.
- Demonstrated strategic planning, population utilization management and the ability to integrate multiple aspects of medical center operations.
- Outstanding communication and organizational skills.


Preferred Qualifications:
- Knowledge of DSM IV 5 axis diagnosis
- Strong experience in assessment, consultation
- Intervention and treatment skills in a HMO outpatient Clinic
- Strong background in crisis, brief therapy, cognitive behavioral and group dynamic required
- Knowledge of the complex systems of psychiatric care and Eating Disorders at the community and county levels; i.e. County inpatient and outpatient services, residential care services, accesses to board and care services, Medi-Cal regulations and Conservatorship laws as they apply to Los Angeles county.
- Effective communication skills
- Ability to work with difficult and high risk population.
- Thorough knowledge of psychiatry and/or addiction medicine-assessment, diagnosis, treatment modalities, psychopharmacology, and institutional care.
- Effective networking and brokerage skills to facilitate placement needs.
- Ability to demonstrate strategic planning, advanced communication skills, negotiation and organizational skills, population utilization management skills and the ability to integrate multifacets of medical center operations.


Notes:
- May require evenings and Saturdays.
- The Behavioral Health Case Manager will provide integrated and coordinated case services for the Eating Disorder population, high risk and high utilizers of the outpatient psychiatry department.
- The content of this position is based on a philosophy of intensive/outreach case management utilizing both brokerage and interdisciplinary team case management models. A major responsibility is to manage all facets of care and be accountable for the reduced utilization of high risk Eating Disorder psychiatric patients.
- The person occupying this position will coordinate services to increase the inter-utilization of such services as outpatient, crisis, partial hospitalization, intensive outpatient, day treatment, group therapy and assessment and diagnosis of Eating disorder Spectrum.
- File intervention and treatment plan-open case File computer retrieval based program
- Environmental Intervention Linkage of member to community services Linkage with KFH-SCPMG services, specifically IOP services Connections with outside contract services, specifically day treatment programs
- Consultation with families and other care givers Maintenance and expansion of social networks.
- Brokerage when necessary
- Collaboration with Health Care Providers Advocacy when necessary
- DirectPatient Intervention Group therapy for chronic, high utilizing patient population
- Individual supportive psychotherapy with client and supportive family counseling when necessary
- Regular participation in Intensive Outpatient programs as a team member, Training in independent living skills Psycho-education, Crisis assessment and intervention Environment Intervention Monitoring progress and quality assurance-release from active to inactive status
- Tracking outcomes for ongoing Quality Management Case Load Estimate 15-25 active cases 25-50 inactive cases Supervisor Supervise, under the direction of Administrative Clinician, LCSW interns and volunteers who will serve as service extenders and perform concrete tasks within programs.
