Is responsible for evaluation, treatment, & referral of members to the appropriate substance abuse program (KP or others). Acts as consultant & liaison to physician & staff as well as to outside facilities/agencies.
Essential Functions:
- Conducts intake evaluations of KP members as appropriate.
- Conducts group & individual counseling for identified substance abuse members & their families.
- Arranges for or conducts follow-up care of substance abuse members.
- Performs record-keeping on substance abuse members in accordance w/ Health Plan medical record standards.
- Refers members to other substance abuse facilities as appropriate.
- Introduces members to Alcoholics Anonymous & Narcotics Anonymous when appropriate.
- Communicates w/ Health Plan physicians & professional staff regarding progress & management of referred members in accord w/ HIPAA guidelines.
- Acts as liaison to & communicates w/ hospital/agency personnel & substance abuse therapists who are providing services for Health Plan members & coordinates w/ local government agencies to provide services for members.
- Coordinates w/ local government agencies to provide services for members.
- Conducts in-service educational programs on the subject of substance abuse.
- Participates in the Planning, development & implementation of a substance abuse program in conjunction w/ the departments of Internal Medicine Pediatrics & After Hours Care.
- Arranges for & coordinates the use of available space for the program & conducts in-service educational programs on the subject of chemical dependency as needed.
- Participates in continuing education programs or seminars in order to remain current in the field of substance abuse therapy.
- Performs other related duties as directed.
Qualifications:
Schedule Details:
Monday, Wednesday &Friday 8:30am - 5:00pm, Tuesday &Thursday 11:00am - 7:30pm.
Basic Qualifications:
- 3 years of experience in direct care of substance abuse clients in an ambulatory setting required.
- Master's degree in Social Work or equivalent degree required.
- LCSW or LPC or LSATP (VA); LCSW-C or LCPC (MD); LICSW or LPC (DC).
- National Provider Identification (NPI) required.
Preferred Qualifications:
- Substance Abuse, Chemical Dependency, or Addictions' certification preferred.
- Experience working in a Chemical Dependency IOP.
- Experience working with dually diagnosed individuals.
- Experience working with Individuals, Families and Groups.
- Thorough understanding of 12 Step Programs.
- Thorough understanding of the Process of Withdrawal.
- Strong Assessment and Treatment Planning Skills.
- Ability to work as a Team Member on an Intedisciplinary Team.
- Crisis Management.
- Bilingual.
