This position serves as the Regional senior leader for and expert in Home Care Services (which includes Home Health, Hospice and Palliative Care) and Continuing Care Services (which includes Long Term Care and SNF). The position is responsible for strategic planning and directing the development, implementation and valuation/improvement of home health, hospice and palliative care services, performance standards, programs and agency operations within the Southern California Region. The position ensures that consistent processes and program objectives extend across all home health and hospice agencies within the Region by collaborating with the Continuing Care Leaders. The seven licensed agencies employ over 1000 FTE's and if freestanding, would rank among the top five nationwide in terms of size and patients served.
Essential Functions:
Strategic Planning
- Works with Medical Center, Continuing Care and Regional leadership to develop home care services strategic plan including development of system value-added services, capacity and resource requirements, and customer needs
- Directs the implementation of the Regional Shared Services' goals and objectives to support the overall functioning of home health, hospice and palliative care programs throughout the region
Leadership and Direction
- Directs the evaluation of the efficiency, cost effectiveness and quality of all home health, hospice and palliative care programs
- Facilitates the Regional Shared Services' functions through appropriate departmentalization delegation of duties per the approved organizational structure
Finance
- Coordinates the annual budget for the Regional Shared Services Unit and the Home Care Services in each Medical Center
- Oversees the Director of Finance's involvement in the development of the agencies budgets in cooperation with the Medical Center's financial departments
- Monitors and evaluates financial performance of home care and addresses variances as appropriate
Policy and Procedure
- Recommends and interprets the need for changes in regional policies and procedures to ensure that home health, hospice, and palliative care are meeting the goals and objectives of each Medical Center and the required standardization of processes and procedures across all agencies
Program Development
- Directs and coordinates all program planning and development for the Regional Shared Services Unit
Personnel
- In cooperation with Medical Centers' administration employs and assigns qualified administrators for each agency. Acts as home health, hospice and palliative care expert in helping to assess appropriate qualifications for personnel hired in each agency.
- Implements good labor relations with all Regional Shared Services staff and supports good labor relations in each agency.
Qualifications:
Basic Qualifications:
- Bachelor's degree in Nursing, Health Services or Business Administration, Public Health, Finance or related discipline such as Public Administration and a master's degree in one of the aforementioned disciplines
- Eight years relevant and progressive management experience to include at least five years' experience managing home health services/programs
- Demonstrated ability to deliver and contract for health care services in continuing care settings
- Indepth knowledge of financial management
- Ability to demonstrate communication, presentation, leadership, analytical and problem solving skills
- Demonstrated ability to lead and manage change through the use of interpersonal skills such as negotiating, collaborating, influencing
- Working knowledge of quality assurance, resource management, health plan regulatory and licensing processes including TJC, Medicare and MediCal regulations related to home care and hospice
- Demonstrated ability to achieve results on both a strategic and operational level
- Demonstrated expertise in project management.
- Must be able to work in a Labor/Management Partnership environment.
