This position provides direct nursing care for a defined population, demonstrating clinical competency by performing various roles as needed, providing advanced levels of nursing care and proactively monitoring and delivering preventive/early intervention and/or education programs to meet patient needs. He or she also provides clinical orientation and ongoing training for nursing staff, including precepting and mentoring co-workers and students.
Essential Functions:
- Demonstrates excellent clinical skills, knowledge and judgement, incorporating continuous learning for self and team that enhances delivery of care and maintains clinical competence. Acts as a clinical and professional role model in all interactions with members and staff.
- Maintains patient flow and coordination of care in daily operations. Fosters collaboration and resolution of team conflict.
- Problem-solves clinical/systems issues that impact patient care. Makes timely, ethical and firm business decisions keeping the customer in mind.
- Manages customer concerns and service recovery.
- Participates in quality initiatives including QIA and HEDIS. Leads designated team projects or initiatives as requested.
- Provides prevention, early intervention, health education and direct patient care according to guidelines, which may include phone and facility encounters.
- Ensures coordination of patient care with QRM, other providers, social workers, community and/or other alternative care interventions.
- Expresses self clearly. Has ability to make effective presentations to small groups.
- Considers success of team and customer needs in decision making.
- Participates in the interviewing and selection of team members that meet the clinical and behavioral parameters of a high performance team. Coordinates orientation and training of new team members.
- Values diversity, fostering respect, appreciation, and inclusion of diversity within the team and the delivery of culturally sensitive care.
- Participates in screening and/or reviews as required.
- Assures team/department's compliance with regulatory agency requirements and federal, state, local laws and regulations.
Qualifications:
Basic Qualifications:
- High School diploma or GED
- Current and valid Georgia LPN licensure
- CPR-BLS certified
- Three years of clinical experience with increasing levels of complexity and responsibility.
- Experience in coordinating the needs of patients/families with complex medical, psychosocial problems.
- Experience in patient education.
- Competency on skills checklist.
Preferred Qualifications:
- Experience in evaluation of quality and implementation of quality improvement initiatives.
