The purpose of this position is to provide training and coding experience using both ICD-9 and CPT-4 coding guidelines to individual who is currently enrolled and in their second year attending an accredited HIM program, such as PCC or COCC or has recently graduated. The individual who will be given guidance and educational opportunities to develop a career in the Health Information Management field as a Certified Facility Coder.
Essential Functions:
- Training and working experience translating clinical information into coded data - Utilizing an encoder software system, analyzing and maintaining it's accuracy, validity, and meaningfulness - Review medical record documentation and assigns ICD-9-CM and CPT-4 classification codes for diagnoses, procedures, physician consultations, and Abstracts and enters clinical data elements as defined by the needs of the organization for patient utilization/outcome studies, financial information, research, reimbursement, and governing agencies
- Training and working experience, validating the accuracy of the clinical coded patient data used for the patient care and financial management for accuracy, consistency, and appropriate assignment
- Sequences diagnoses and procedures by following the Uniform Hospital Discharge Data Set (UHDDS), and adhering to standards of billing set by Medicare (HCFA), Medicaid (OMAP), and other intermediaries
- Ensures the accuracy of data input into the regional health information management system; communicates with hospital coding staff to resolve coding discrepancies and/or documentation issues concurrently; interacts with physicians to clarify and accurately document patient diagnostic and procedural information
- Meets department standards for performance and quality
- Training and working experience utilizing electronic patient data system and clinical information system (EpicCare) to access patient encounter information
- Monitors data entries of members receiving services at plan hospital sites, assuring all patient coverage types are appropriately identified, and updating patient database as necessary - Performes additional healthcare data abstracting to meet KP's database standards for patient and physician profiling and utilization
- Audits accuracy of admissions and discharges daily, editing patient admission/discharge/transfer database as appropriate; problem-solve data entry and system interface issues
Qualifications:
Basic Qualifications:
- Prior experience in Facility Coding or an accredited HIM qualifying program, or closely related work setting with course completion of basic medical terminology, anatomy and physiology with disease processes, and ICD-9 and CPT-4 education - Current enrollment and in their second year attending an accredited HIM program, such as PCC or COCC or recent graduate - Working knowledge or education that includes basic instruction in the classification and coding of healthcare information for reimbursement, statistical, and research purposes - Demonstrated working knowledge of disease processes, diagnostic and surgical procedures, ICD-9-CM and CPT classification systems, and medical terminology - High school diploma or GED
- Fluent in English - Demonstrated ability to use independent thought and judgment and to easily adapt to unfamiliar automated systems - Demonstrated ability to effectively communicate, verbally and in writing
Preferred Qualifications:
- Coding experience with demonstrated knowledge of medial terminology, anatomy and physiology, basic knowledge of medical procedures and disease states/processes - Current enrollment and in their second year attending an accredited HIM program, such as PCC or COCC or recent graduate - Combination of education and experience, of disease processes, medical terminology, diagnostic and surgical procedures, ICD-9-CM and CPT classification systems, and health information/medical record department responsibilities - Fluent in English - Documented ability to use independent thought and judgment and to easily adapt to unfamiliar automated systems - Able to process work, error free, within prescribed deadlines and productivity standards
Salary Range:
$17.39-$22.99
