The Physical Medicine Team Lead assures delivery of high quality, cost-effective care to Kaiser Regional Rehabilitation and Kaiser Sunnyside patients by acting under the direction and in partnership with Regional Rehabilitation Services Management to provide clinical leadership for the physical medicine staff.
Essential Functions:
- The Physical Medicine Lead will assure delivery of high quality, cost-effective care to Regional Rehabilitation Services patients by:
- Assisting and participating in the hiring and orientation of physical medicine staff.
- Assisting with oversight of staff performance, providing input to staff evaluations, and identifying and reporting problems that impact delivery of patient services.
- Assuring that physical medicine staff are supported by acting as a liaison to Regional Rehabilitation Services Management, Facilities, and the Northwest region by conducting physical medicine meetings.
- Providing oversight of day-to-day staffing and patient schedules. Prioritizing and problem-solving visits and referrals with the Member Intake Specialists.
- Collaborating with Regional Rehabilitation Services Management by participating in appropriate policy/procedure development, representing the physical medicine staff at meetings, and by participating in physical medicine equipment purchase decisions and maintenance plans.
- Serves as a member, representing the physical medicine team, of the Regional Rehabilitation Services Leaders Meeting in the absence of the PT/OT Supervisor.
- Assuring inter-department coordination between the Regional Rehabilitation Services Physical Medicine group and other groups within the Region by acting as a liaison to other departments within Kaiser Permanente and outside agencies.
- Leads case conference/team meetings as appropriate.
- Provides skilled therapy services to patients consistent with practice and licensing standards under a care plan directed by the client's physician.
- Exhibits professional behavior supporting regional and departmental policies, procedures goals and values.
- Activities include performing initial and ongoing assessments of patient rehab potential, functional needs and abilities, activity tolerance, mobility, equipment needs, compliance with physician ordered care, psychosocial status, need for community resources and rehabilitation services.
- Actively seeks opportunity for continued professional growth and knowledge.
- Develops and implements an individualized care plan utilizing departmental and community resources in a cost-effective manner.
- Skilled plan of therapy care includes therapeutic exercises/activities, modalities, adaptive equipment based on patient assessment and understanding of treatment risks and benefits relevant to the patient's medical condition, rehabilitation potential and prognosis.
- Coordinates the delivery of therapy care with nursing, social work, aide and therapy to Kaiser Permanente and non-Kaiser Permanente providers which results in effective utilization of resources and the timely attainment of achievable rehabilitation.
- Supervise therapy assistants through on-site evaluation of the provision of care, conferencing and review of their documentation according to State Practice Act and agency policy/procedure.
- Provide supervision of the rehabilitation aide plan of care within state/federal guidelines.

Qualifications:

Basic Qualifications:
- Five (5) years of physical therapy, occupational therapy, or speech language pathology experience.
- For Lead Therapist position minimum two (2) years of recent experience as a therapist in the ambulatory or hospital setting.
- One (1) year of leadership (preceptor, supervisor, or manager) experience or commensurate experience.
- Experience as an interdisciplinary team member.
- Prefer some budget development and management experience.
- Bachelor's of science in physical therapy OR a certificate OR master's degree in physical therapy OR bachelor's of science OR master'sdegree in occupational therapy.
- Graduation from a physical therapy program approved by the American Physical Therapy Association OR the Council on Medical Education and Hospitals of the American Medical Association in collaboration with the American Physical Therapy Association OR Graduation from an American Occupational Therapy Association-approved program.
- Licensed in Oregon and/or Washington. Requires license in primary state (Oregon or Washington) prior to first day of employment.
- Valid driver's license.
- Basic Life Support CPR card for Health Care Provider. Valid card must be obtained prior to patient contact and within two weeks of hire.
- NPI.
- Must be able to safely assist patients with mobility and transfers.
- Demonstrates competency in the use of therapy modalities and equipment.
- Demonstrates effective interpersonal communication.
- Able to prioritize patient needs to achieve attainable treatment goals.
- Thorough knowledge of teaching/learning principles.
- Working knowledge of interdisciplinary care coordination.
- Keyboarding or typing skills.
- Good communication skills required.
- Working knowledge of the managed care industry preferred.
- Knowledge of Medicare regulations/requirements preferred.
- Knowledgeable in the care for the injured, chronically ill, and frail elderly is preferred.
- Excellent time management, problem solving and customer service skills preferred.
- Excellent written and verbal communication skills preferred.
- Ability to work autonomously and manage their caseload is preferred.
- Operational knowledge of computerized clinical documentation and office application systems preferred.
