Practices professional nursing by the performance of both independent nursing functions & delegated medical functions in accordance w/ accepted practice standards. Such functions include the initiation & performance of nursing care through health promotion, supportive or restorative care, disease prevention, diagnosis & treatment of human disease, ailment, pain, injury, deformity, & physical or mental condition using specialized knowledge, judgment, & skill involving the application of biological, physical, social, & behavioral science principles. Actively seeks information to understand member/patient circumstances, problems, expectations, & needs. Builds rapport & cooperative relationships w/ members/patients. Considers how actions or plans will affect members; responds quickly to meet member/patient needs & resolves problems.Specialty Responsible for providing palliative care, in close collaboration w/ an interdisciplinary palliative care team, to patients w/ an advanced illness. Supports & coordinates the comprehensive & effective delivery of Palliative Care Services, according to scope & assigned tasks, in various settings, including inpatient program, home-based program & outpatient clinic program. - Collects subjective & objective data
- Analyzes reports, & record data
- Uses data to identify health care problems & create plan
- Triage patients either in person or via telephone & implements proper course of action in accordance w/ established protocol/guideline or in consultation w/ provider
- Conducts telephone calls backs to patients requiring follow-up care
- Evaluates emergency situations & initiates appropriate nursing interventions
- Health teaching & health counseling to patients and/or their families
- Specialty Position Responsibilities may vary according to program service & may include, but are not limited to the followingPerforms multidimensional assessment of members w/ advanced illness, to determine & coordinate the best level of care
- Assists the patient/family in identifying personal goals of care, preferences for end-of-life care, & promoting communication of advanced directives
- Collaborates to build strong, effective relationships w/ referrers, their teams, & w/ partner agency care staff to assist in development & communication of plans for co-management of patients
- Teaches palliative care modalities to providers, referral sources & members
- Documents member care planning & program outcomes
- Assists in maintaining data for monitoring program outcomes
- Assists in identifying patients appropriate to the palliative care department
- Attends interdisciplinary team rounds w/ partner agencies to assist w/ interventions for complex patients, in conjunction w/ the Palliative Care MD & team
- Acts as liaison/interface w/ KP providers & resources within the system
- Identifies possible problems in all care delivery systems which care for our palliative care patients
- Assists w/ development & implementation of solutions for service providers & staff
Qualifications:
Basic Qualifications:
- Minimum of 3 years experience as an RN, w/ at least 2 of the 3 years experience caring for chronically & terminally ill adults & their families
- Excellent patient interviewing & assessment skills
- Competencies include pain & symptom management, emotional, psychosocial, & spiritual/ cultural understanding, honoring patient & family care wishes & needs regarding end of life & bereavement
- Applicant must be a graduate of board approved RN program & completed a minimum of 750 clock hours of faculty planned clinical experience & guided learning activities which required direct supervision by faculty, associate nursing instructional personnel (ANIP) or preceptor who is physically present or immediately accessible
- Currently licensed as a RN in the State of Colorado
- National Provider Identifier (NPI) & Taxonomy code required at time of hire
- Demonstrated customer service skills, customer focus abilities & the ability to understand KP customer needs
- Familiarity w/ databases & word processing programs
Preferred Qualifications:
- Palliative care and/or hospice experience preferred, including effective collaborative team practice skills
- CHPN (Certified Hospice & Palliative Nurse) or equivalent preferred & desired within 2 years of hire
Preferred skills -
hospice or palliative care experience
