Essential Functions:
ALL -
1. Conducts telephone call backs to patients requiring follow-up care.
2. Evaluates emergency situations and initiates appropriate interventions.
3. Health teaching and health counseling to patients and/or their families.
4. Collaborates effectively with all members of the care team.
5. Maintains clinical competence through attending staff development programs, is responsible for maintaining professional certification.
6. Builds rapport and cooperative relationships with members/patients
7. Considers how actions or plans will affect members; responds quickly to meet member/patient needs and resolves problems
8. Participates in planning for the delivery of clinical service
9. Actively seeks information to understand member/patient circumstances, problems, expectations, and needs
10. Participates in the peer review process of not only your own professional qualifications but those of your colleagues.
11. Participates in training of students
12. Makes members/patients and their needs a primary focus of one's actions; develops and sustains productive member/patient relationships
13. Carries out clinical and administrative procedures that may be promulgated by the Health Plan
14. Participates in monitoring quality of care according to professional standards and established criteria
15. Facilitates the coordination and integration of care between health care services; manages populations of patients to ensure appropriate utilization of health care resources.
16. Review and monitor therapy and treatment patient plans. Initiate appropriate consultations and/or actions accordingly.
17. Work as a member of the healthcare team
18. Maintains established systems of follow-up for patient care
19. Performs other related duties as directed
20. Executing delegated medical functions which deliver aspects of care that implement and are consistent with the medical plan as prescribed by a licensed or otherwise legally authorized physician or person acting under the physician's delegated authority.
Nurse Practitioner

21. Collects subjective and objective data. Analyzes, reports, and records data. Uses data to identify health care problems and create plan. Prioritizes patient's needs. Triages patients either in person or via telephone and implements proper course of action
22. Evaluating health status through the collection and assessment of health data CRS 12-38-103 (10). Examines and treats patients, orders and interprets diagnostic tests and makes diagnosis.
23. Practices within scope of practice perimeters as inferred by licensure, standard nursing practice, knowledge, skill level, sound clinical judgment, and KP guidelines.
24. Accurately documents patient data, actions or interventions, responses, and plans for care/follow-up according to guidelines and/or standard nursing practice.
25. Providing therapy and treatment that is supportive and restorative to life and well being either directly to the patient or indirectly through consultation with, delegation to (per Colorado State Board of Nursing rules and regulations for delegation), supervision of, or teaching others.
26. APN specializing in surgery may provide preoperative and postoperative care. As first or second assistants during major surgery
27. Refers to medical or community agencies per Kaiser Permanente guidelines those patients who need further treatment, evaluation, or assistance.
28. Leads health care team by influence and role modeling integrated effective nursing practice, exemplary service, innovation and providing outstanding support for physician practices.
29. Collaborates with articulated plan physician, and follows Health Plan policy around annual chart review and discussion between APN and physician
30. Participates and cooperates with colleagues both other Advanced Practice Nurses and Physicians in evaluating professional qualifications
31. Patient advocate.
Physician Assistant
21. Provide medical care under the supervision of physicians and in accordance with Physician's Assistant education, training, and certification, within delegated guidelines, established practices and procedures or specific instructions.
22. Participates and cooperates with colleagues; both other Physician Assistants and Physicians in evaluating professional qualifications
23. Maintains ongoing supervisory physician agreement
24. Makes telephone evaluations as indicated, as well as giving medical advice and information to patients by telephone where appropriate
25. Compile medical histories, examine and treat patients, order and interpret diagnostic tests, and make diagnoses. May instruct and counsel patients and order or carry out therapy.
26. Record actions and information in the progress notes of the patient medical record.
27. PAs specializing in surgery may provide preoperative and postoperative care and may work as first or second assistants during major surgery.
28. Collaborates with the physician and other clinical staff members as needed
29. Refers patients to supervising physician or, with physician agreement, to other health professionals as appropriate
Qualifications:
Basic Qualifications:
ALL
BLS required within 90 days of employment and must be maintained as current.
National Provider Identifier (NPI) and Taxonomy code required at time of hire.
DEA Registration (or actively working to obtain this registration if new graduate)
For positions requiring the administration of procedural sedation, ACLS certification and procedural sedation competency required within 90 days of hire.
Must obtain and maintain credentialing and/or privileges both internally and externally by the affiliated hospital. Ongoing credentialing required.
Nurse Practitioner
Currently licensed as a Registered Nurse in the State of Colorado. Listed on the Advanced Practice Registry and is in compliance with those requirements. Must be a graduate of a RN program that meets Colorado State Board of Nursing curriculum requirements.
Masters Degree in Nursing
Graduation from a NLN accredited Nurse Practitioner program
Prescriptive authority required or in progress. Must be obtained within 2 years of hire date if a newer graduate
For positions in Pediatrics or a position as family nurse practitioner in a pediatric setting, education and certification required for. Pediatric nurse practitioner or Family Nurse Practitioner for a Pediatric setting,
Physician Assistant
Bachelor's degree required. Graduation from an program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA)
Current and on-going certification by the National Commission for Certification of Physician Assistants (NCCPA)
Preferred Qualifications:
One year of primary care experience preferred
This position will be rotating Monday-Friday, will require coordinating coveragewith other midlevel provider.
External hires must pass a background check/drug screen.
