Collaboratively define, develop, execute, evaluate, manage and improve the cross-functional health system to exceed customer service expectations in providing high quality, evidence-based services to members and community partner members with chronic conditions to achieve market-leading performance of strategic significance to the region.
Essential Functions:
- Seek out opportunities for innovation and apply evidence evaluation principles in collaboration with national and regional clinical, business and service evidence experts, clinicians and operational leaders, to collaboratively, objectively, and critically evaluate evidence and define cost benefit regarding emerging and new technology, innovation, best practices, and employer group expectations and incentives, to assure responsible stewardship of health system resources in planning and providing safe and effective market-leading chronic condition services.
- Actively participate in and inform leadership discussions to define the Region's strategic positioning and performance targets for services which support populations with chronic conditions. (Currently Population Strategy Group)
- Collaboratively define an evidence-based stakeholder-supported strategic operational plan which supports KPNW's long-term viability while optimizing existing systems and tools, defines modes and prioritization to provide care for members with chronic conditions, key department roles, resources needed, personnel responsible, timeframes, and appropriate patient populations to produce excellent customer experiences while providing high quality, safe, effective, efficient market-leading chronic condition services.
- Coordinate with clinicians, operational departments, medical office managers and staff, and other stakeholders to engage and collaboratively implement, manage, execute, problem-solve, define performance metrics, define performance, evaluate, communicate and continually improve the services while creating a broad culture of ownership, accountability, compliance, ethics and integrity which consistently provides high quality, highly satisfying, patient-centric, memorable experiences and market-leading chronic condition services.
- Hire, train, coach, supervise, and recognize chronic condition staff.
- Share best practices and collaborate with other managers and leaders to facilitate coaching and recognition of staff involved in providing chronic condition and surveillance services throughout the region.
- Actively communicate strategies, execution plan and performance updates to internal audiences, employer groups and other external audiences.
- Collaboratively develop promotional materials and serve as resource to facilitate region-wide operational, communication and marketing consistency to facilitate excellent member experiences in chronic condition care.

Qualifications:
Basic Qualifications:
- Five (5) years of clinical practice experience.
- Five (5) years of work in health care systems in a coordinating function.
- Two (2) years of leadership experience.
- Demonstrated experience and effectiveness in engaging work teams to implement change, improve services, and improve performance.
- Demonstrated experience in cross-functional program development, evaluation, measurement and improvement.
- Bachelor's or doctorate degree or degree in clinical profession (RN, NP, Pharm.D., BS. Pharmacy, RD).
- Highly effective interpersonal communication skills and group process that demonstrate ability to collaborate with clinicians and interdisciplinary teams and administrative staff.
- Demonstrated skill in leading and delivering results through influence, change and efforts of others.
- Knowledge of evidence, therapeutic strategies to achieve clinical targets
- In depth appreciation of line management accountabilities.
- Personal influence and credibility.
- Demonstrated effective skills in complex, large scale project and change management in an integrated health care system.
- Demonstrated proficiency in customer service, change leadership, performance, execution, strategic systems thinking and cultural competence.
- End user computer capability.
Preferred Qualifications:
- More than three (3) years of direct experience implementing Regional systems preferred
- Five (5) years of clinical practice experience preferred.
- Direct experience in managing health care services preferred.
- Master's degree in systems, business or health care profession preferred.
- Licensed to practice nursing or pharmacy preferred.
- Development, implementation, evaluation and continuous quality improvement of a new department, service or function preferred.
- Thorough knowledge of the care of patients with chronic conditions, roles of various health care team members, and multidisciplinary care at Kaiser Permanente or other large integrated health system preferred.
- Knowledge of region's information systems preferred.
- Understand basic principles of financial and systems analysis preferred.
- Financial, service and quality analysis preferred.
- Expert communication, group process, organizational development and conflict resolution skills preferred.
