For members of a defined population, responsible for collaborating w/ the members of the health care team to facilitate the coordination of appropriate, cost-effective services that are consistent w/ member's plan of care, help achieve his/her optimal level of independence, & enhance quality of life.
Essential Functions:
- Responsibilities include, but are not limited to, problem identification, psychosocial assessment, financial counseling/referral, accessing community resources, placement for care, guiding the member through health-related legal processes, or consultation & support to other health care professionals.
- Effectively manages & coordinates assigned caseload consistent w/ established criteria. Completes comprehensive psychosocial assessment to evaluate patient goals, social support systems, resources, health status, functional limitations, psychological status, environmental factors, & response to treatment so as to decrease inappropriate utilization of medical services.
- In close collaboration w/ the nurse case manager & other members of the health care team, develops & monitors a plan of care designed to promote the member's optimal level of functioning & enhance the quality of life.
- Identifies, facilitates, & advocates appropriate organizational & community resources to meet the plan of care & ensures that they are implemented for in a cost effective, efficient, & timely manner.
- Ensures consistent & reliable documentation of case management activities in compliance w/ all organization & department standards.
- Analyzes patient & program outcomes to identify improvements in program, quality, & cost effectiveness of case management activities.
- Facilitates application process for accessing local, state, & federally funded programs (e.g., Medicaid, Medicare, & Disability) &/or refers to appropriate community agencies in cases of suspected patient abuse/neglect when identified.
- Provides supportive counseling & education to members, families & caregivers, members of the health care team, health plan staff, & the community, including end-of-life issues & Advanced Directives.
- Promotes self-awareness & knowledge of current case management standards in the community & recent innovations in patient care. Maintains current knowledge of laws, regulations, & policies relating to the practice of social work in the local market/local agencies & maintains high social work standards as defined by the NASW Code of Ethics.

Qualifications:

Preferred Skills: Experience with chronic renal failure and peritoneal dialysis paients.

This position will travel to all NOVA medical buildings.

Basic Qualifications:
- 3 years of clinical experience plus 2 years in case management required.
- At least 1 year of experience w/ the defined population required.
- Master's Degree in Social Work (MSW) required.
- Social Work Clinical licensure sanctioned by the State to which assigned required.
- Case management certification (CCM) required (or acquired within three years of employment in this position).
Preferred Qualifications:
- Experience w/ computer software programs in a Windows environment preferred.
- Knowledge of community systems & resources in the defined service area preferred.
- Knowledge of regulatory issues for the Mid-Atlantic area preferred.
