Functions as Case Manager for High Risk/Oncology patients. Develops and coordinates programs and care plans relating to patient care. Evaluates and treats High Risk/Oncology patients. Provides in-service education to staff. Functions as IV Therapy Management Coordinator in outpatient setting. **Note: For the purpose of this job description, 'High Risk' patients are defined as including but not limited to asthma, CHF, HIV, diabetes and other chronically ill patients who need case management services to maintain a basic level of health and functionality to prevent deterioration.

.Essential Functions:
- Supports physician in planning and developing follow-up care/services for High Risk/Oncology patients. Coordinates care and services with outside agencies, in-house departments, and hospitals. Serves as liaison between internal and external care providers.
- Assesses, evaluates, and treats High Risk/Oncology patients by formulating and/or implementing care plans. Under physician direction, administers chemotherapy. Orders, records, interprets, and reports laboratory and other tests.
- Provides case management services for High Risk/Oncology patients to insure quality of care, consistent treatment regimens and early intervention. Serves as liaison between patient panel and PCP. Formulates and coordinates care plans to assure continuity of care. Receives and handles phone calls from patients and their families regarding care problems and other pertinent information, and triages to appropriate person/department.
- Assesses patient's home support systems. Coordinates and monitors services provided by community agencies and institutions. Makes home visits as indicated.
- Provides Ambulatory Infusion services to targeted patients. Teaches and supports patients and their families/care givers in learning to care for their family members. Assists patients/families with identified problems/concerns. Clarifies patient/family expectations and guides in redirection.
- May perform patient care to the extent necessary to maintain clinical expertise, competency and licensing necessary to fulfill job responsibilities and to direct the provision of care on the unit.
- Provides direct patient care on an as needed basis. Provides services that are within scope of license and in compliance with all legal, regulatory, and policy requirements relevant to clinical role performed.
- Incorporates the KP Nursing Vision, Model and Values through out their Nursing Practice.
Qualifications:

Basic Qualifications:
- Two years experience working with chronically ill patients.
- Case Management/Home Care experience.
- Bachelor's degree in nursing; or equivalent related experience.
- Valid Hawaii RN license (must meet education requirement(s) for Hawaii State licensure).
- Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification.
- Valid driver's license
- Demonstrated skill in mastery in administering IV medications/solutions and inserting IV and PICC lines, Medicare and Medicaid coverage issues and managing a panel of patients.
- Demonstrated knowledge of and skill in oral communication, written communication, change management, adaptability, customer service, interpersonal relations and teamwork.
- Demonstrated knowledge of and skill in word processing, spreadsheets and database PC applications.

Preferred Qualifications:
- Certified Diabetes Educator (CDE) by the National Certification Board for Diabetes Educators (NCBDE).
- Oncology Certified Nurse (OCN) by the Oncology Nursing Society (ONS).
- Knowledge of Community resources and available services.
- Familiarity with oncology medications and protocols.
- Knowledge of Kaiser Health Plan benefits and system.
