This position works under supervision of a Clinical Pharmacy Assistant Manager as designated. They support Formulary adherence by providing consistent medication review and benefit coverage determination. Additionally, he or she develops and maintains criteria addressing Non Formulary medication benefit coverage and appropriate alternative medications.
Essential Functions:
- Interacts with practitioners, peers and patients in order to effectively educate and provide prescribing process guidance and benefit coverage determinations.
- Partners with Chief of Pharmacy physician and Affiliated Care physician to promote compliance with the Non Formulary process as well as collaboration regarding benefit coverage criteria and when addressing difficult benefit coverage determinations.
- Partners with medical office building pharmacy assistant managers to ensure pharmacy compliance with Non Formulary process and drug initiatives.
- Performs accurate medication reconciliation for patients receiving transitional care (hospital, SNF, LTCF discharge orders), with the goal of timely, appropriate medication utilization during all transition points.
- Assembles trend data as well as drug or criteria specific data for presentation to the P and T committee. Responds to requests from P and T committee or Pharmacy Quality Council.
- Supports and designs opportunities to attain regional strategic initiatives (ie. Prescription cost PMPM; Interregional Drug initiatives) and document interventions.
Qualifications:
Basic Qualifications:
- Pharm D or Bachelor's of Science in Pharmacy
- Georgia Pharmacy License (or ability to obtain one prior to time of employment)
- Motivated to expand pharmacy knowledge base and comfortable researching drug topics
- Skilled communicator. Persuasive professional verbal and written communication skills when interacting with practitioners, peers and patients
- Demonstrates comprehensive knowledge of Kaiser Permanente drug benefits, sufficient to effectively mentor others
Preferred Qualifications:
- Board Certified Pharmacotherapy Specialist
- Previous experience in specific pharmacy service areas (e.g. drug benefit and formulary management) preferred
- Knowledge of formulary systems, pharmacy operations, pharmacy benefits management
- Demonstrates extensive experience in formulary adherence
