Under indirect supervision, performs dispensing pharmaceutical care including patient consultation (OBRA 90), and supervisory duties pertaining to filling of prescriptions. Approves orders during filling process and oversees assigned subordinates. Assures optimum service to patient, physician or other authorized individuals, within federal and state regulations. Works as interdisciplinary team member in support of patients, providers, and ancillary staff to ensure appropriate, cost-effective drug utilization while maintaining and enhancing patient care. Provides pharmaceutical care by performing various clinical, educational, informative, and distributive functions.
Essential Functions:
- Prepares and dispenses medications using appropriate techniques and following appropriate policies and procedures
- Oversees or performs receiving and review of prescriptions and orders assuring recorded information meets all legal requirements, preparation of medications using appropriate techniques and following applicable departmental policy and procedures; obtains clarification on all questionable prescriptions or directions
- Authorizes dispensing of completed prescriptions and provides patient consultation (OBRA '90)
- Designs, recommends, monitors, and evaluates patient specific pharmacotherapy
- Collects and organizes patient-specific information needed by pharmacist to prevent, detect, and resolve medication-related problems and make appropriate medication therapy recommendations
- Provides concise, applicable, and timely responses to requests for drug information from healthcare providers and patients; accurately identifies requester's drug information need; formulates systematic, efficient, and thorough procedure for retrieving drug information or referring/utilizing appropriate resource services
- Provides medication-use education to patients and caregivers that effectively meet their needs
- Uses effective patient education techniques to provide counseling to patients and care givers, including information on medication therapy, adverse effects, compliance, appropriate use, handling, and medication administration
- Provides direct oversight for functions and activities of support personnel to assure responsibilities are performed in accordance with scope of practice and applicable procedures
- Prioritizes workload and organize workflow, taking into account available resources
- Checks accuracy of support personnel by following established departmental procedures
- Actively participates in monitoring and improving relevant outcome measures and/or quality improvement indicators
- Participates in regional medication management programs and formulary education programs to promote appropriate, cost-effective, prescribing through effective communication and education with prescribers and members
- Provides information concerning availability of new, alternate or similar drugs and information on incompatibilities, interactions, contraindications of prescribed drugs to physician, RN, or others
- Demonstrates sufficient knowledge, skills, and abilities in area of specialization
- Demonstrates knowledge, skills, and abilities necessary to provide care and/or service appropriate to age groups served
- Performs other duties and accepts responsibility as assigned

Qualifications:

Basic Qualifications:
- Graduate from an accredited pharmacy college recognized by the American Council of Pharmaceutical Education (ACPE)
- Completion of an emergency contraception training course approved by the American Council of Pharmaceutial Education (ACPE), curriculum-based programs from an ACPE-accredited college of pharmacy, applicable state or local health department programs, or programs recognized by the board of pharmacy
- Valid Hawaii Pharmacist license (must meet education requirement(s) for Hawaii State licensure)
- Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification
- Demonstrated ability to safely handle and/or prepare various pharmaceutical compounds (e.g., chemotherapeutic agents, biologicals)
- Demonstrated knowledge of and skill in adaptability, change management, conflict resolution, customer service, decision making, interpersonal relations, oral communication, problem solving, quality management, teamwork, and written communication
- Demonstrated knowledge of and skill in word processing, spreadsheet, and database PC applications
- Talking to co-workers, customers, outside vendors, and on the telephone
- Reading, writing, speaking, understanding English
- Training/giving and receiving instructions
- Mathematical ability, attention to detail (e.g. organization, prioritization, proofing), concentration, and alertness
Preferred Qualifications:
- Doctor of Pharmacy Degree from a school recognized by the American Council of Pharmaceutical Education
- Completion of a general residency program
