Position Highlights:
The RN Program Coordinator for Pre Kidney/Pancreas Transplant actively provides supervised patient care through a diverse role as a clinician, educator, and consultant in a financially responsible manner.
The nurse works in a collaborative relationship with other members of the health care team to provide quality and consistent care.
Position Responsibilities:
Coordinate the movement of adult and pediatric patients through the referral and evaluation process, pre-transplant management and listing process/organ allocation
Initiates the medical evaluation of the potential donor and recipient: coordination of diagnostic testing, tissue typing, referrals to physician, review of patient medical records
Responsible for identifying the learning needs of pre-transplant patients, implementing teaching plans, and documenting patient plans and progress
Coordinate the recipient and living donor evaluations
Serves as a resource for the multi-disciplinary health care team
Demonstrate an understanding of the certification process and financial approval
Monitors patient outcomes in accordance with defined standards of patient care
Provides education to health care personnel and community groups
Coordinates and organizes formal symposia to promote advances in transplantation
Participates in Organ Donor Offer Calls and Kidney Distribution
Position Qualifications Include:
Bachelor’s degree in nursing preferred
IL Nursing License
Bilingual-Spanish
CPR certification
2-3 years med/surg or acute hemodialysis experience required
1 year experience in management of organ failure and transplant patients preferred
Knowledge of diseases of the kidney and pancreas
Excellent verbal, written and communication skills required
