You may be the right candidate for our Transplant Nurse Coordinator!

Position Highlights:

  • The RN Program Coordinator for Pre Kidney/Pancreas Transplant actively provides supervised patient care through a diverse role as a clinician, educator, and consultant in a financially responsible manner.

  • The nurse works in a collaborative relationship with other members of the health care team to provide quality and consistent care.

Position Responsibilities:

  • Coordinate the movement of adult and pediatric patients through the referral and evaluation process, pre-transplant management and listing process/organ allocation

  • Initiates the medical evaluation of the potential donor and recipient: coordination of diagnostic testing, tissue typing, referrals to physician, review of patient medical records

  • Responsible for identifying the learning needs of pre-transplant patients, implementing teaching plans, and documenting patient plans and progress

  • Coordinate the recipient and living donor evaluations

  • Serves as a resource for the multi-disciplinary health care team

  • Demonstrate an understanding of the certification process and financial approval

  • Monitors patient outcomes in accordance with defined standards of patient care

  • Provides education to health care personnel and community groups

  • Coordinates and organizes formal symposia to promote advances in transplantation

  • Participates in Organ Donor Offer Calls and Kidney Distribution

Position Qualifications Include:

  • Bachelor’s degree in nursing preferred

  • IL Nursing License

  • Bilingual-Spanish

  • CPR certification

  • 2-3 years med/surg or acute hemodialysis experience required

  • 1 year experience in management of organ failure and transplant patients preferred

  • Knowledge of diseases of the kidney and pancreas

  • Excellent verbal, written and communication skills required
