Serves as a single point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within Kaiser Permanente, Northwest. This position assures the patient has the necessary appointments across the continuum for individual patient, including those made with lab, diagnostics areas and specialty physicians.
Essential Functions:
- Assists patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical research studies and technologies.
- Responsibilities include assessment, analysis, planning and implementation of interventions.
- Serves as an essential link between patients and all other care providers in coordination of care.
- The breast care navigator facilitates changes in the plan of care and brokering necessary services.
- Service settings may include the hospital, outpatient, or home.
- The care is assessed and conducted by phone.
- Facilitate appointments for consultation and support services within established services standards including cancer counseling, dieticians and genetic counselors.
- Coordinate and standardized patient education tools across the services of plastic surgery, general surgery, radiation therapy and medical oncology.
- Responsibilities include gathering up.
- Follow the patient throughout the continuum of care, including inpatient admission and collaboration with inpatient.
- Work with Cancer Registry staff and others to collect data, track outcomes, and support strategic planning processes.
- Adhere to established productivity standards.
- Responsible for networking to maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, inpatient nurses, radiology staff, counseling staff, and radiation oncology staff, other specialty care staff.).
Qualifications:
Basic Qualifications:
- One (1) year of oncology experience.
- Minimum two (2) years of experience in acute care or ambulatory care/clinic setting within the last five (5) years.
- High school diploma or GED.
- Graduate of Accredited School of Nursing.
- Current RN license in Oregon and Washington. Requires Oregon or Washington license at time of hire. Licenses in both states are required within 6 months of hire.
- OCN within six months of hire.
- Current BLS w/AED certification.
- Demonstrates clinical nursing and leadership skills.
- Able to type 20 words per minute.
- Demonstrates effective interpersonal, communication and problem-solving skills.
- Willingness to learn computerized information systems.
- Demonstrates ability to work within teams and within a dynamic work environment.
- Customer focused.
Preferred Qualifications:
- Previous Breast Cancer Navigator experience, radiation oncology, surgical oncology experience preferred.
- Previous experience with population care/case management, triage and advice preferred.
- MSN preferred.
- Breast Cancer Navigator certification preferred.
Salary Range- $33.10 - $47.60
