The Non-traditional predictors of rehospitalization and high resource use (NTPR) study will define a parsimonious predictive model that could be used operationally to identify patients at high risk of rehospitalization. By analyzing three data sources, we propose to determine whether we can identify a small set of variables that could be combined with existing electronic data so as to improve model discrimination. The sources are published studies on rehospitalization predictors, existing KPNC and non-KPNC databases, and data from prospective patient interviews. Our patient population will consist of adult KPNC members hospitalized between 1/1/08 and 12/31/14, with prospective data restricted to patients hospitalized in 2013-2014.

Conducts telephone or in-person interviews with study participants. Recruits and consents study participants. Assists in training new study staff in study protocols and provides instruction to study participants. Uses computer software to provide basic data management and generate reports.

Essential Functions:
- Assists in training new study staff in protocols and implementation of study goals/objectives
- Provides instruction to participants to teach them how to perform specific study protocols
- Prepares computer-generated reports, including statistical tables
- Answers participants' and providers' questions
- Screens and recruits participants; obtains consent
- Schedules examination appointments for study participants
- Conducts telephone or in-person interviews with study participants and providers
- Reviews questionnaires for completeness and accuracy; checks for inconsistencies; and codes open-ended questions.
- Assists with all aspects of mailed surveys
- Monitors participant's progress (e.g., blood pressure, exercise, response to questionnaires, tracking, etc)
- Provides basic data management using database software
- Maintains all pertinent project records and files
- Transcribes data from records to forms

Secondary Functions:
- Recruit patients at medical centers in Oakland, San Francisco, Walnut Creek, and Hayward, to participate in the NTPR Study.
-Screen study participants by communicating with hospital staff.
-Conduct study visit, which includes conducting informed consent and administering questionnaires.
- Obtaining informed consent from study participants.
- Conducting in-person interviews with study participants. Respond to participant questions and requests. Administer, collect, and edit questionnaires. Prepare forms for data entry. Review questionnaires for completeness and accuracy; checking for inconsistencies.
- Assist with preparation of recruitment and visit materials.
- Participant monitoring, follow-up, and tracking activities.
- Maintain all pertinent project records and files.
- Entry of data into study tracking systems and assist with data cleaning. Maintain subject and data tracking systems.
- Inform the Principal Investigator and Project Manager of the status of project operations and any problems encountered.
- Assist in training new study staff in protocols and implementation of study goals/objectives.
- Shift varies, scheduled days and hours are contingent upon departmental needs. This position may include some evening and weekend hours.
- Perform other project-related tasks, as necessary.
Qualifications:

Basic Qualifications:
- Minimum of one (1) year experience in a research/health care environment required
- Previous interviewing experience required
- Experience in editing/coding questionnaires required
- Experience and knowledge of computer applications, such as word processing and database software, required
- Associate's degree or equivalent experience required
- Bachelor's degree or equivalent experience preferred
- Bachelor's degree may substitute for experience in field
- Excellent interpersonal and communication skills; telephone skills required
- Familiar with medical terminology
- Must be able to work in Labor and Management Partnership environment

Preferred Qualifications:
-Ability todemonstrate excellent organizational, interpersonal, and communication skills.
- Previous recruiting and field interviewing experience preferred.
- Ability to work successfully with a wide variety of internal and external project staff and study participants.
- Ability tobe dependable, punctual, detail-oriented, and able to follow detailed protocols precisely.
- Ability to exhibit a professional manner with a high degree of courtesy, tact, and sensitivity in particular with patients who are hospitalized and severely ill.
- Ability to beable to work well independently, as well as part of a larger multi-disciplinary research team, with support from Project Manager and Principal Investigator.
- Knowledge of computer applications, such as word-processing and database software, required. Proficiency with Microsoft Office products (Word, Excel, Access, and PowerPoint) highly desirable.

NOTE: Travel is required. Must have own car and valid California driver's license. Must be willing to travel between Division of Research to medical centers in Oakland, San Francisco, Walnut Creek, and Hayward.
