To receive and provide accurate information in regards to opening medical leaves/disability claims for FMC employees and ensuring that leave information is provided to appropriate plants and FMC representatives in a timely manner.  The Representative is dedicated to providing the best in class customer service to all UniCare disability customers ensuring the uniform delivery of service to all our customers by explaining medical leave package information and related procedures.

 

DUTIES AND RESPONSIBILITIES:

  • Answer incoming calls in accordance with company standards
  • Confirm that caller is requesting a conditional medical leave of absence and establish if this is a new leave, ineffective or change in claim status i.e. NWA to total disability.
  • Obtain pertinent information to open medical leave if absence is the result of an accident or injury that is work related, must advise employee to contact work location, get confirmation from employee that contact was made or will be made
  • Let employee know that his/her plant will be notified of the 14 day conditional leave request and that employee must supply their work location with medical justification using the form 5166 and that the location will assess medical leave eligibility.
  • Explain the documents contained in the medical package and employees responsibility as it relates to the documents
  •  Advise when the leave package will be mailed
  • Open Disability Claim.
  • Ensure that all medical leave information is sent via Email to employee’s work location with all pertinent information the same day.
  • Complete merge letters as required
  • Load leaves information from worksheet into the database and
  • Follow proper protocol for responding to telephone inquiries from claimants, providers benefit representative, etc.
  • General claims support duties as needed
  • Other duties as assigned

  • Prepping 50-75 files per Month
  • Internal Audits 95%
  • MLA Audits 97%

High School Diploma or GED required
Customer Service Experience
Strong Computer Skills required
Excellent oral communication skills required
Ability to work independently and effective decision making with supervisory support required
